EXPERIENCE

Chantel is a nurse with over 14 years of experience in trauma-surgical care, burns, plastic surgery, ENT (including adjusted airways), and district nursing. She has established and managed a community care service regulated by the Care Quality Commission.

She has collaborated closely with Integrated Care Boards (ICBs) for clients with complex needs, such as adjusted airways, tube feeding, and skin integrity issues. As a case manager, Chantel handles diverse caseloads, conducts assessments, and recommends safe community management strategies, working with other health professionals to ensure holistic care.

Her extensive experience includes managing multidisciplinary teams and ensuring client safety in the community. She actively participates in the Continuing Healthcare (CHC) process, from Decision Support Tool (DST) assessments to appeals and engages in high-level discussions on care services.

In managing multifaceted cases, Chantel lead multidisciplinary teams (MDTs) to provide cohesive and comprehensive care for clients with diverse needs. She prioritises a goal-based approach, setting measurable outcomes to guide interventions and track progress, fostering accountability and alignment with the client's care plan.

Additionally, she is  skilled in managing budgets to optimise resource allocation without compromising care quality. Her role involves identifying clients' personal goals and ensuring client-centred care plans. By incorporating therapeutic leisure activities, she has observed notable improvements in clients' quality of life, providing meaningful engagement and a sense of achievement.

PERSONAL PROFILE

Chantel enjoys staying active in her free time by attending yoga classes, skiing, running and walking with her dog. Spending time with her family, especially in the outdoors, is a cherished hobby. They often explore nature together, which provides a refreshing break from their daily routines.
